Refer A Patient

We have provided care to many hundreds of people and do all we can to support our patients and their loved ones at the most difficult time of their lives.
Our services are available to patients registered to a North or Mid Beds GP and are offered Monday to Friday.
Referrals can be made by Health or Social care professionals, by a carer or family member or by the patients themselves.
